No outstanding membership fees and any disciplinary fines, plus one months membership fee to cover administration costs, as detailed in the membership sign up form (copy of this clause is listed below for reference). 2). Return of all club kit to the team manager. Process Once form is received, the club secretary's will check that fees have been paid up to date and that the kit has been returned to the team manager. An invoice for one months fees will be sent to cover administration costs and this is to be paid within 14 days. Upon receipt of this, we will then cancel the players registration and fee account. Please allow up to 14 days for this to be actioned. Parent/Guardian Signature* (Use your mouse/pointer on a laptop/PC or your finger on mobile or iPad devices): Termination of Membership Clause: This is taken from the Membership Form for the 2022/23 Season: Should your child exit the club during the calendar year (August – April) all subs shall be paid up to the date of transfer or exit plus one month to cover administrative costs. Should your payments have fallen into arrears prior to exit or transfer the club (FCAM) reserve the right to withhold a transfer request until such a time arrears are cleared and report the debt to Wiltshire County FA. In the event of exiting the club where arrears are outstanding the club, (FCAM) reserve the right to inform Wiltshire County FA / and or a debt collection agency, which could affect your child’s ability to sign for a future club. Also on exit or transfer all FCAM property is to be returned or the above action will be applied.
